Could anybody tell me where from this sloka about Ganesha is taken. I will be very grateful for the correct translation of the firts 2 lines.

vighnAnan darantu hErambha charaNAmbhujarE namaha
Ekadantam mahAkAyam lambOdara gajAnanam vighnanAshakam dEvam
