regarding dhyana slokam pujya shri anantharama dikshithar us

the slokam was a composition of number of slokaas taken from various slokams and it also contained slokams such as "mookam karothi vaachalam lankam pangayathe giram..." and last slokam was "bhuddhir bhalam yasho dhairyam...". can anybody tell what that slokam was. my late mother used to sing the whole slokam but i lost the opportunity that i did not get that in writing when she lived.
