Sonali,

Here is my attempt:
OmkAra vAkyam uraga pungava bhUSitAngam
vyAghrAjinAmbaradharam jaTinam trinEtram
pAshAnkushadharam abhayakarapradam shUlapANim
kailAsa bhUdharapatim praNatOsmi nityam
praNatOsmi nityam praNatOsmi nityam

I worship/bow down (praNatOsmi) every day (nityam) to him, who is the very embodiment of the OmkAra (OmkAra vAkyam), whose limbs (angam) are adorned (bhUshita) with the most excellent (pungavam) of snakes (uraga), who wears (dharam) garments (ambara) made of the skin (Ajina) of a tiger (vyAghra), whose hair is in the form of matted locks (jaTinam), who is three-eyed (trinEtram), who holds (dharam) both a noose (pASa) and a hook (ankuSa), who hands (pANim) hold a lance or trident (Sula), and who grants (pradam) a vision of his hand (kara) in the abhaya (fearlessness) mudra, who us the lord (patim) of mount kailAS, that supports (dhara) the earth (bhU).
