the first word would be jagadAchAryashankarebhyaH (n was missing)

Translation:
P
The Moha-mudgara ( the hammer to shatter delusion) is obtained by Shankaracharya ,the preceptor to the world.
A
The forest of delusion is destroyed, and from that, the real nature of things, is seen.
C1
Just as moths fall into fire, humans fall into sense-objects in vain, having lost their power of discrimination due to the darkness of delusion. Therefore, acquire the Moha-mudgara!
C2
When one is freed from the net of delusion, the wrong perception of permanence of the impermanent, purity in the impure, and happiness in what is sorrow-filled, perishes. The real nature of the Self is understood.

Rajani mahodaye, Thank you. Wonder who is the composer.

प्रायः एवं भवति खलु —
शङ्कराचार्यकरेभ्यः/शङ्करेभ्यः respectful plural (पञ्चमी) मोहमुद्गरः (मया/अस्माभिः) लभ्यते we get the hammer to destroy our delusion from (the hands of) Adi Shankara.

?
Just my thought.

Thank you Savitri ji.

SAchi Sir, very important additions from you. Mohamudgara is indeed another name for Bhajagovindam
Pattamma - please read the translation as : "is obtained from", and correct the Mulam as "bhrAntirnaSyati"
